Glen Park partners with Five Star Home Hospice, Inc. and Steward Hospice Care, Inc. to help those in our care and their families to live as fully as possible when facing the end of life. We believe regardless of age, race, handicap, sex, sexual orientation or national origin, that everyone has the right to control how they choose to live at the end of life. After all, this is a time in life when Moments Matter Most!
Five Star Home Hospice, Inc. is a state-licensed, Medicare/Medical-certified, and JCAHO-accredited hospice located in Pasadena providing 24 hours/7 days a week hospice care to patients with terminal illnesses, while giving emotional support to their families. We service the Greater Los Angeles area and are dedicated to providing high quality end-of-life care to all persons in need of those services. At Five Star Home Hospice, we believe that a patient with a life limiting illness has the right to live his/her final days comfortably, to the fullest extent possible, and to experience a dignified and peaceful passing.
Hospice care is appropriate for patients of all ages who are homebound or not, and who have a life-limiting illness including:
- Cardiac Disease
- Lung Disease
- Kidney Disease
If a person is experiencing any of the following symptoms, hospice may be a viable option to increase quality of life:
- Uncontrolled or increasing pain
- Increased shortness of breath
- Increased difficulty in swallowing
- Progressive, uncontrolled swelling
- Unresolved nausea / vomiting
- Oxygen dependence
- Mounting kidney and urinary difficulties
- Numerous hospitalizations and ER visits
Our hospice is well-equipped to serve the multicultural population of Southern California. We have staff fluent in: Spanish, Mandarin Chinese, Taiwanese, Armenian, Russian, Tagalog, Arabic, Farsi, Turkish, and French.
When medical treatments can no longer cure a disease, Steward Hospice Care, Inc. provide an individualized program of physical, emotional, spiritual, and practical care for persons in the last stages of a life-limiting illness. Our palliative care program focuses on quality of life (comfort and dignity) for terminally ill people and is centered on both the patient and their family. We provide care wherever people can benefit from hospice care – their home, in a skilled nursing facility, and assisted living / residential care facility.
We recognize that end of life is one of the most difficult events that a person and family must face. We will do everything in our power to earn your trust and we will always strive to be honest and straightforward in all matters that relate to our patients and family members.
Our hospice care is available to patients and family – 24 hours a day, 7 days a week. On-call. We have staff on standby who make visits, respond to sudden changes in condition, answer any questions, order medication, oxygen, or medical equipment, and provide emotional or spiritual support.
Individuals who are eligible for Steward Hospice Care’s services do not have to be homebound but may experience some or all of the following:
- Frequent hospitalizations
- Progressive weight loss
- Deteriorating mental abilities
- Recurrent infections
Steward Hospice Care’s clinical staff is qualified and able to assist patients whose diagnosis includes any of the following diseases that, generally, result in hospice care admission:
- Adult Failure To Thrive (Multi-system Failure)
- Adult Immune-Deficiency Syndrome (AIDS)
- Amyotrophic Lateral Sclerosis (ALS or Lou Gehrig’s Disease)
- Cancer (of the Bladder, Breast, Brain, Colon, , Liver, Lung, Lymphoma, Ovarian, Pancreas, Prostrate, Stomach)
- Cardiac/Heart Disease (Cardiomyopathy, Cerebral Vascular Accident – CVA, CongestiveHeart Failure – CHF or Stroke)
- Dementia (Alzheimer’s Disease)
- Parkinson’s Disease
- Pulmonary Disease (COPD)
- Liver Disease
- Death Inducing Trauma such as a Car Accident
Hospice care is covered by Medicare, Medi-Cal, and most private insurance; therefore, there are no out-of-pocket expenses for eligible patients or their families. We provide services throughout Los Angeles, Riverside, San Bernardino, Ventura County, and Orange County.
