Five Star Home Hospice, Inc. is a state-licensed, Medicare/Medical-certified, and JCAHO-accredited hospice located in Pasadena. We service the Greater Los Angeles area and are dedicated to providing high-quality end-of-life care to all persons in need of those services. At Five Star Home Hospice, we believe that a patient with a life-limiting illness has the right to live his/her final days comfortably, to the fullest extent possible, and to experience a dignified and peaceful passing.

Our agency prides itself on being able to provide a fast response time to requests for hospice. We are a small hospice agency able to provide individualized attention to our patients. Upper management is committed to meeting with all patients and families. Our field and office clinical staff have an average of 20 years of experience in the healthcare field and have the clinical knowledge to manage all types of hospice patients. We provide a team-oriented approach that addresses all aspects of comfort care, including excellent medical care, pain, and symptom management, and emotional/spiritual support expressly designed to meet the needs and wishes of the patients and their families.

Our hospice is well-equipped to serve the multicultural population of Southern California. We have staff fluent in Spanish, Mandarin Chinese, Taiwanese, Armenian, Russian, Tagalog, Arabic, Farsi, Turkish, and French. Five Star Home Hospice has strong spiritual and psychological resources for our clients. We have multicultural and multi-denominational clergy, and social workers from a variety of backgrounds.

Our Promise at Five Star Home Hospice is to meet the needs and standards of our terminally ill patients, their families, and loved ones, with dignity and compassion by providing Five Star palliative (comfort) care to improve the quality of their lives, focusing on the emotional, social, physical and spiritual support that they need. Care is provided for the patient and family 24 hours a day, 7 days a week. A Five Star Home Hospice nurse is always available to answer your questions and help you.

When medical treatments can no longer cure a disease, Steward Hospice Care, Inc. provides an individualized program of physical, emotional, spiritual, and practical care for persons in the last stages of a life-limiting illness. Our palliative care program focuses on quality of life (comfort and dignity) for terminally ill people and is centered on both the patient and their family. We provide care wherever people can benefit from hospice care – their home, in a skilled nursing facility, and assisted living / residential care facility.

We recognize that end of life is one of the most difficult events that a person and family must face. We will do everything in our power to earn your trust and we will always strive to be honest and straightforward in all matters that relate to our patients and family members. 

Our hospice care is available to patients and family – 24 hours a day, 7 days a week. On-call. We have staff on standby who make visits, respond to sudden changes in condition, answer any questions, order medication, oxygen, or medical equipment, and provide emotional or spiritual support. 

Individuals who are eligible for Steward Hospice Care’s services do not have to be homebound but may experience some or all of the following:

  • Frequent hospitalizations
  • Progressive weight loss
  • Deteriorating mental abilities
  • Recurrent infections

Steward Hospice Care’s clinical staff is qualified and able to assist patients whose diagnosis includes any of the following diseases that, generally, result in hospice care admission:

  • Adult Failure To Thrive (Multi-system Failure)
  • Adult Immune-Deficiency Syndrome (AIDS)
  • Amyotrophic Lateral Sclerosis (ALS or Lou Gehrig’s Disease)
  • Cancer (of the Bladder, Breast, Brain, Colon, , Liver, Lung, Lymphoma, Ovarian, Pancreas, Prostrate, Stomach)
  • Cardiac/Heart Disease (Cardiomyopathy, Cerebral Vascular Accident – CVA, CongestiveHeart Failure – CHF or Stroke)
  • Dementia (Alzheimer’s Disease)
  • Parkinson’s Disease
  • Pulmonary Disease (COPD)
  • Liver Disease
  • Death Inducing Trauma such as a Car Accident

Hospice care is covered by Medicare, Medi-Cal, and most private insurance; therefore, there are no out-of-pocket expenses for eligible patients or their families. We provide services throughout Los Angeles, Riverside, San Bernardino, Ventura County, and Orange County.
